Direct-to-R2 presigned PUT. Large files bypass the Worker body.

  1. POST /api/media/presign → returns uploadUrl + r2Key (short-lived, ≤10 min)
  2. Client PUTs file directly to R2 (bypassing Worker)
  3. POST /api/media/confirm with r2Key + sha256 → pending → ready
